Appointments

The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Cancellation Policy                                                                                                                                                                                       

Your scheduled appointment time has been reserved specifically for you.  Cancelled or missed appointments with less than 24 hours notice will be charged a $75 fee.  

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements. 

     

Financing Options

In addition to submitting insurance claims on your behalf we are dedicated to making your services as easy and affordable as possible. We feel it is very important for everyone to be educated and informed about the procedure or procedures that they are interested it. We also want to make sure that you are well informed about the cost and payment options available to you. That is why our Treatment Coordinator will gladly sit down with you and assist you in deciding which form of payment will work best for you and your individual needs.
We accept cash, check, visa and master card. We also accept Care Credit which offers up to 12 months interest free financing.  Care Credit also offers low interest extended plans for up to 60 months for those who need help fitting larger treatment plans into their budget.

Insurance and Billing

If you do not have dental insurance you are responsible for payment in full at the time of service.  If you do have insurance, your estimated portion is due at the time of service.
We are a contracting provider with Delta Dental only.  Although we do not contract with other insurance companies, as a service to our patients, we do process claims for most insurance carriers.  We will gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services provided.
You can expect your insurance company to reimburse you for a portion of our fee. That portion is determined by the contract between you and your insurance company. We do not recommend treatment based upon insurance coverage but we will try to help you maximize your benefits.
