Cancellation Policy

Your scheduled appointment time has been reserved especially for you. Canceled or missed appointments with less than 24 hours' notice will be charged a $75 fee.

Patient Forms

If you are a new patient to our office, please download our new patient bundle with forms that will need to be filled out before your appointment. Printing them, filling them out, and bringing them with you will allow us to attend to your medical needs more quickly than completing them on arrival. Thank you, and don't hesitate to contact our office if you have any questions. If you would like to fill out the forms electronically, give us a call and we will send you a text or email with the link.

Payment Options

We accept cash, personal checks, money orders, and most major credit cards for your convenience. Payment is required at the time services are conducted. Feel free to contact our office if your treatment plan requires more extensive dental work and you want to discuss financial arrangements.

Financing Options

In addition to submitting insurance claims on your behalf, we are committed to making your services as easy and affordable as possible. We feel it is essential for everyone to be educated and informed about the procedure or procedures that they are interested in. We also want to ensure that you are well informed about the cost and payment options available. That is why our Treatment Coordinator will gladly sit down with you and assist you in determining which form of payment will work best for you and your individual needs. Insurance and Billing

If you do not have dental insurance, you are responsible for the full payment at the time of service. If you have insurance, your estimated amount is due at the time of service.

We are a contracting provider with Delta Dental and Aetna. Although we do not contract with other insurance companies, as a service to our patients, we will file claims with your insurance company for you. We will gladly provide dental x-rays and a written diagnostic report should your insurance company have any questions about the services.

 We do not recommend treatment based on insurance coverage, but we will try to help you maximize your benefits.
